Requirements

Learn the requirements needed to become one of our recepients

Potential recipients of funding from the Putnam First Cancer Fund must be a resident of Putnam County, Florida AND Have no insurance, be underinsured, or be medically needy, and/or have a financial need.

 

Potential recipients will have to provide photo identification, proof of residency in the county, proof of income for the last 30 days, and a breakdown of charges for typical monthly expenses including copies of bills, etc, if any.
